This website is a library for all resources related to the new Capital & Coast Early Warning Score (EWS) & Vital Signs Charts introduced in September 2015. These are presented in a variety of formats, most of which are not editable. These systems are in use across both hospitals within Capital & Coast District Health Board in Wellington, New Zealand. Both adult & paediatric (PEWS) charts are presented below. Please note that there are 5 paediatric charts which are age specific.

  • The Adult EWS was developed by Alex Psirides & Anne Pedersen during 2015 to update the Wellington EWS & bring it in line with current evidence.
  • The Paediatric EWS was developed by Sinéad McCarthy, Charlotte Stanczuk, Alex Psirides & Anne Pedersen & is based on charts originally designed by Canterbury DHB.
  • The vital signs charts were designed by Alex Psirides, Anne Pedersen & Elena d’Eon.

NEW EARLY WARNING SCORE MATRIX

This graphic shows the scored parameters, their assigned scores & the banding zones they fall into. Extremes represent the Medical Emergency Team (MET) calling criteria for which an urgent ‘777’ call must be placed.

Adult Escalation Pathway

WELLINGTON MANDATORY EARLY WARNING SCORE ESCALATION PATHWAY

This graphic shows the mandatory escalation pathway for patients with an EWS of 1 or more. A patient scoring 10 or more or with any vital sign in the blue zone must have an immediate ‘777’ MET call placed.

EWS Activation Sticker

Early Warning Score Activation
EARLY WARNING SCORE ACTIVATION STICKER

This sticker is designed to be placed in the medical record of any patient who triggers an EWS activation. It allows audit of response times of reviewing medical staff.

Should be printed at A4 size on paper with an adhesive background. The linked document contains 4 EWS stickers which should be divided before use.

MET Activation Sticker

Medical Emergency Team Activation
MEDICAL EMERGENCY TEAM ACTIVATION STICKER

This sticker is designed to be placed in a patient’s medical record by the team at the end of a MET call. It highlights to the primary team that their patient had required the attention of the MET; as such the colours chosen are particularly offensive so as not to go unnoticed. This is particularly important for patients seen by the MET out-of-hours where the primary team may be absent.

Should be printed at A4 size on paper with an adhesive background. The linked document contains 2 MET stickers which should be divided before use.

This form can be used to collect data at every MET call. In Wellington, it has since been superceded by an electronic database but is preserved here for historical reasons & may be of use to others.

Interhospital Patient Transfer

Patient Record Interhospital Transfer
INTERHOSPITAL PATIENT TRANSFERS

This form is used by the Wellington Flight Retrieval Service as an interhospital transfer record. It incorporates information to help with transport planning as well as integration of the EWS into the vital signs record in-transit. The record also provides data on all retrieval times so any delays can be identified as part of Flight Retrieval Service governance processes.

PAR Referral

PATIENT AT RISK TEAM REFERRAL RECORD

This form is used to record patient information at the time of referral to the Patient At Risk (PAR) team. The PAR team are expert nurses with critical care skills, available 24/7 in Wellington Regional Hospital. They act as a key component of the pre-MET system. The form follows the 'SBAR' recommendations for referral. All information is subsequently entered into an online database.

These provide additional information around the introduction of the new charts & EWS.

  • The eLearning presentations will form the basis for self-directed learning through the Intranet; They are available here in Powerpoint format for ward teaching for both the Adult & Paediatric EWS systems.
  • The Medical Bulletin has been distributed to all junior medical staff & provides the rationale for the new adult EWS as well as explaining how the medical components of the chart should be used (particularly the ‘EWS Modifications’ box).
